I completed the campaign on Legendary Co-Op with my friend BrutalBagel45 (I am only saying his GamerTag because he got the achievements and I didn’t) and didn’t get the achievements. I got the armor and the Legendary visor, but no achievements.
I went back to the campaign and it says I have only completed 5 missions! That is not true. I have achievements for all 9 missions, and the armor to prove I’ve completed the campaign.
Anyone else have this problem?
Yes I did. Is it possible that you were disconnected from Xbox Live perchance? I don’t know whether the achievement is just on Solo or also available for Co-Op. Not sure. Might be a glitch.

It happened to a friend but he fixed it deleting his gamertag and downloading it again, hope it works.
