I had purchased/preordered my Halo 4 Limited Edition console bundle via EBGames last year, but had never received any Fotus in-game DLC armor like the many other users out there.
I’ve already received the run around from Xbox Live support to come to the Halo forums to address my query(they said they can’t help me, despite them having helped others by sending out extra codes to those whom didn’t receive it initially).
Either my console bundle had a packaging error and just didn’t come with the code, or Canadians weren’t entitled to it at all? Was the code only exclusive to U.S. and UK customers? I don’t see why I should have to get it off eBay for ripoff prices when it should have come with my bundle in the first place. So I’m stumped on this one.
Is there any officials here on the forums I can contact to discuss this issue? Any Admins or other contacts I can be forwarded to?

The box that the console came in should have had a card with a set of 4 or 5 DLC codes on. Each code was for a different thing, with the top code being the one for all the Fotus armour and emblems.
Nope… not my box. I even looked to see if it was hidden inside the owner’s manual packaging… The only card I get was the externally included promo card from EBGames which gave me the HAZOP, Battle rifle skin and all specializations unlocked.
My code was in the game case that was packaged with the console.
I bought 2 consoles, one had the card/insert with the codez. The other console did not. So I contacted MS. I had to provided all the information I had and a copy of the receipt for the console that didnt have the insert. A few days later I got the codes emailed to me. I highly suggest contacting them.
I’m sorry to hear the code card wasn’t included. I think the best thing you can do is try taking it back for a refund.
I don’t think anyone at 343 will be able to give you the codes, as that would probably open the floodgates and they’d be inundated with people claiming to have bought it but not receive the codes.
Unfortunately some people would try lying about it in order to get them for free.
