I’m having the same issue, I’m not even able to launch the game, I’ve tried cleaning disc, turning off and on and nothing seems to work. I’ve played others games with no issue.
P.S my disc is not scratched nor anything wrong with it.

My issue doesn’t require a disc. I have the game downloaded from the Marketplace and I have messages sometimes popping up saying that my disc is unreadable, which is weird because I don’t need a disc to play Halo 4.
Your problem is probably just having a corrupt disc.
This is actually an odd issue that occasionally happens with downloaded games. You’ll want to try some of the steps listed in this thread on the Xbox Forums.
If the solutions listed there are not helping, you’ll want to reach out to Xbox Support.
