How do you submit your forge map that you made to 343 so they can test it?
You don’t, they are always looking at the maps passed around between the community the most, which obviously become the most popular. If you want your map out, you’l have to get the community to talk about it first 
You can’t.
343i has no official way of testing maps, so it’s nearly all luck whether your map is even seen.
