Me and a couple friends did the Halo 3 LASO playlist to try and get the “Preference for Pain” achievement, and after a while we finally beat it! Unfortunately when we beat the playlist it says it’s fully completed but none of us got the achievement. We did the reach LASO playlist already and had no issues with that one. I’ve tried to look up solutions and the only consistent things I find are replay the epilogue and/or opening cutscene if possible or play the whole playlist again, which I definitely am not doing.

Update: The achievement randomly propped up when I was playing DMC 5
