I am trying to play with a friend and when he appears he does but he is dead and cannot revive. Did someone turn?

I think what this should be on MCC Support.

But be sure of not being attacked or jumping and meleing meanwhile is dead.

When does this happen in the game? Is he able to spawn in when you start the mission?
