On the mission The Oracle during the cutscene before the heretic leader boss fight it boots to main menu.
On the mission Sacred Icon the cutscene to Quarantine Zone doesn’t trigger causing a soft-lock

Have had the same issue during The Oracle during the cutscene where the heretic leader locks himself in the safe room, and on Delta Halo where the player first gets the warthog towards the beginning of the mission. Has been occurring for well over a year now.
