I bought the limited edition halo 4 xbox and game and was suppose to come with FOTUS skin code and did not. How do I get this, I have already spoken to xbox and they recommend this site.
If I’m right, your store should have provided you with the code if it wasn’t inside the console’s box.
You’d have to take it up with them.
Alternatively, you could try sending a PM to bs angel or rukizzel

What The Little Moa said is correct but you must remember that BsAngel and Rukizzel get a lot of messages everyday so it might take a while to get a reply from them.
If you purchased the Limited Console, and didn’t receive codes, indeed speak to the store customer service/manager, explaining the issue and like Moa said, perhaps they can give you a new code or refund. There is no guarantee though, if they’ll give you a code. You may have to exchange it for a new console and hope and prat the codes are in there.

I’ve never heard of anyone getting missing codes from this site - your best bet is to contact the store you bought it from.
Had the console previously been opened and then returned to the store before you bought it? Someone may have taken the card with the codes on it.

Indeed. I happened to some guy at my school. He bought the console from gamestop around christmas, and when he opened it, nothing. Turns out it was used when he got it, not new.
