I’ve tried a lot of stuff. I redownloaded the map to my console. Reinstalled the title update even. It seems no matter what I do, when I go into Halo 4 offline or sign out, pull the internet plug, and sign back in offline, the console immediately goes into Alzheimers Mode and forgets that I even have it on my HDD.

Well its the same with Spartan Ops, its technically on the disk and as is the downloaded Episodes 6-10, but you can’t play it offline.
Some of this game has pure online components, and if the rumors about the next Xbox are true, get used to it.

It’s not on the disc though. If I remember correctly, I can access Forge Islands without internet connection. I don’t know how to fix this, OP, but I wish you luck on getting it fixed. 
Forge Island is not on-disk DLC but I know I can play it offline because others can as well.
Do you have it saved on the cloud by any chance? If it is even possible to do that it would be a reasonable explanation.

Nope, it’s not. But not to worry. I just fixed the problem. I can play it offline now. Thanks very much to everyone who took the time to read this annoying troubleshooting thread and respond.
And for those unfortunate few who are also getting the same problem as me and are also looking for a solution, it turns out it was actually my custom privacy settings on my account getting in the way. A quick switch back to the default Adult setting returned everything to normal. An obscure cause indeed, but not the first time I’ve heard of custom privacy settings messsing things up.
Case closed.
