This happens all the time when I try to assassinate someone, and it happened to me, good thing I thought fast!

Ok, 343 directed sarcasm aside, that’s happened to me several times. I think it’s caused when the victim is on the high ground.

Or (this has happened to me) you drop down on someone and hit them in the back for the assassination, but there isn’t enough room.
