Games Halo 5: Guardians

Hello, whenever i attempt to play Halo 5 Guardians it comes up with a message that reads ‘FireTeam left. Your fireteam has changed’ this is now preventing me from finishing a game and has cause me to recieve a banhammer. This is extremely thrustrating and i hope this issue is seen to.
