So I was playing Reach a few weeks back, and as it happens, my teammate killed me on purpose. It didn’t give me the option to boot him. He did it again, still no option. It happens another 2 times before I decided to kill him back (IDK why) but I killed him one time, and I was booted by him. Why? Why didn’t it give me the option to remove him from the match? And WHY ARE THERE THOSE TYPE OF PEOPLE!?!?!?
this doesn’t just happen to me in Reach though. It has happened in Halo 3, and in Halo 4 (I think).
Griefers like the person you mentioned just suck. Period.
Yeah I don’t know why the system is like that. In my opinion it needed more fine tuning.
I have a feeling that he was killing you in a way that wasn’t obvious to the ban hammer so he wouldn’t get banned.

No I was stuck by plasma grenades. Killed by rockets, shot in the head during SWAT, shotgun, and just straight punched. There are others but those come to mind for non accidental
I was killed by a Teammate by accident and it gave me the option to boot him twice.
