Just got halo 4 for Christmas, both disks seem in excellent condition. Here’s what happened:

Put in disk 1, everything was fine but campaign wouldn’t load. It would just go back to the main screen.

Installed disk 2 as suggested by fellow gamers. Disk 2 installed just fine.

Put back in disk 1. Try to play campaign and although it loads, the disk now becomes unreadable.

Cleared hard drive and re-installed disk 2. Tried disk 1 again. Disk is suddenly unreadable.

Cleared hard drive again and re-installed disk 2 again. Disk loads campaign again, but becomes unreadable shortly after.

Tried multi-player. War games worked for 1 match, then the disk became unreadable.

Next step is to exchange disk but I’ll have to do it tomorrow.
Xbox 360 is an original 2007 white xbox 360 that runs other games smoothly with no issues. I have tried a few games in it this morning and it runs them fine.
343, has any progress been made on determining the cause of these issues? It seems many have had seemingly faulty disks like mine only to have them exchanged to find these problems persist. I will try and exchange it tomorrow but if there is still a problem I don’t know what to do. I will also try and see if it works on a friends xbox 360.
Quite frankly, this is totally unacceptable. I’ve seen games come out with game breaking bugs before, but even those games can usually run. Halo 4 is not even working for me. What gives?
