Anyone else do the disc exchange with their Limited Collector’s Edition’s of Halo 5 and only receive the standard disc with the standard packaging and no REQ or Fall of Reach codes?

Yep, same situation. Oddly enough, the codes are missing on my end as well.
Microsoft should have e-mailed you with the DLC codes.
That being said, the DLC code they sent me didn’t have the Req bundle or Fall of Reach either…
Contacted Microsoft today about the missing content. They gave me a code, worked fine, but now I am unable to find the Warzone REQ Bundle in my Games & Apps like the Halo 5 dashboard is telling me though it is confirmed I own it. Told Microsoft about that, they ran me through some troubleshooting but no luck. I have a case open with them. Anyone else having this issue?
The Fall of Reach stuff is all on the Halo Channel BTW.
