Dear 343, My clan uses the flaming skull emblem as the main emblem, so today we were going to grind for the spartan slayer pack. However i had already completed this commedation, and never recieved the emblem.
The Flaming skull emblem comes from the spartan spree commendation; not the spartan slayer one. You need 29 more multi kills to unlock it.
