Every episode is done and only one is on normal difficulty the rest are heroic or higher.

I had the same problem, but I solved it by replaying Spartan Ops Episode 1 Chapter 1 on easy.

Not sure why, but it works. Probably a programming glitch. However, I think the 343 Industries team will probably address the issue in a following update.
Complete any Spartan Ops mission again, regardless of difficulty level. It will unlock then.
Episode 5 is bugged. It will at some point “forget” that you have completed episode two. I think this is why the cheevo is not popping, because at the end in the final mission it doesnt think episode two is completed so no cheevo. As soon as you do another SO mission after episode 5 it updates again and corrects itself.
I had the same issue. Completed all on heroic through the “find Games” and it didn’t pop. Went Back and completed “The Cauldron Base” on Normal and got it. A glitch for sure, but minor and an easy fix.
I have this problem, I thought I didnt get it because my Game black screened on the last part of Ep 5 Chapter 5 so replayed it and still it didnt pop, So read this post and did Ep 1 Chapter 1 on easy and it just popped so its all good

Yet ANOTHER glitch. 343i is really testing their luck.

Yeah, it doesn’t matter what chapter you do, just make sure you complete it. It wouldn’t unlock for me so I replayed chp 1 ep 2.
