In the beginning of Two Betrayals cutscene, Chief puts the index inside the control room, and then Cortana shows up on “desk” and shows the index in digital form, and then the index disappears, did Cortana take it for herself or did she erased it?
She took it, as seen in halo 3.

Yeah, she hung on to it but it was pretty much forgotten about until she revealed she still had it and used it to activate Installation 04b.
