This person claims to have gotten it from completing the Halo 4 campaign on 360.

You unlock the Mark VI Gen 1 helmet and armour automatically if you’ve beaten any campaign mission in the Master Chief Collection. You also get the scarred variant of the helmet if you beat the Halo 2 mission ‘Cairo Station’ on MCC.

I know they’ve been working on MCC so maybe they fixed something and a glitched achievement unlocked. I should point out that I don’t know if completed achievements still unlock the reward if they’re glitched or if you already completed that achievement.

I beat a MCC mission so that must be it
