I was gone for a few days and my stepbrother was playing on my gamer tag. When I returned home I notice that I was banned from voice for one week. I looked online at www.halo4.com/ban and I believe it was not for any of the reasons listed on that page.
The reason I am posting this is to find out possibly why the ban was placed on me and how it works. My gamer tag is DJGryph, I appreciate any information as to why the ban was placed.

The voice ban is completely player controlled, if somebody mutes you X times within X amount of time, expect a voice ban.
Moa’s right. To clarify things a bit:
In-game voice bans are given when a certain number of players have muted you in-game within a set period of time. The system does not try to validate mutes: a mute will bring you closer to a ban even if you don’t have a mic plugged in, are in Party Chat, or are in other circumstances that prevent others from hearing you.
