Problem solved by turning vibration off.

Did you try switching off vibration?

Sometimes batteries are too small for XBOX controllers, try DIFFERENT BRANDS.

I think your controller is messed up.And wow at that pic posted by the user who posted above me.

Turn off vibration in your controller fixes everything and even reduces controller lag.

I’ve had times when it wouldn’t let me throw a grenade, sprint, or melee.

Your controller has so low charge remaining that it can’t really support the vibration anymore. Change your batteries, same has happened to me several times.
