Were you online for all missions? If not, then complete those missions online.
Mark IV obtained if you complete the Commendations > Player > Campaign section. Since commendations unobtainable offline, you need to complete them online. You did everything right, but you need to be online to get it.
Check your campaign commendations first. Make sure you played all the missions online.
