For the longest time I only needed 4 achievements in CE Anniversary, One for Campaign Completion on Heroic, One for Campaign Completion on Legendary, One for Killing all of the Elites on Keyes, and One for Finishing with a Minute left on the Maw. I started playing a few days ago and I only had to do the last 4 mission (Library to The Maw) (I had to do them split screen cuz that’s how I started them.) I only did one mission per play session and after I would complete it, it’d say I completed in on Legendary Coop (which is right) but then once I’d close the game and play the next time it’d say that I only ever completed it on normal. I even have video evidence cuz I streamed the library and Keyes. I don’t understand why this is happening and I feel essentially locked out of the last two achievements I need. Maybe it’ll work if I do all four of the missions in one session??? But then that’s gonna take about 3-4 hours and that’s not even a guarantee that I’ll get them.
PS: I forgot to mention that I got the one for killing the elites on Keyes and the one for one minute left on the maw but not the other two.
Do you mean with the in-game “difficulty completion” tracker?
This issue happens when for some reason your Xbox save data and your Xbox Cloud saves as well as Halo Waypoint become out of sync, usually the result of a network hiccup either your end or Microsoft’s end.
You can rectify this but it will take some effort on your part. Do the following:

Check on Halo Waypoint which missions are showing as incomplete on Legendary. MCC on Xbox is incorrect. These are the missions you will ultimately have to replay.

Attempt to re-do the missions.

If this does not unlock them, you can try a partial factory reset of your Xbox (keeps saves and game installs)

If the above does not work, then a full factory reset of your Xbox will force a resync. This solved my problems with MCC (twice) and DOOM 2016. WARNING this will erase all of your save data on the Xbox and uninstall all your games. You MUST make a backup of any save data you wish to keep before doing this.
I can confirm the final step worked for me but it is time consuming and understandable if you don’t wish to do it. There may be other steps you could try before the full factory reset so I advise you to try other options first but as an absolute last resort, it will work and fix the issue for you. What will then happen when you start MCC, is the game will re-sync your cloud save data. Use the Cloud save data and NOT the backed up Xbox save data. This will then allow you to earn achievements again and the Legendary missions will track.
I had to do this for Halo 4 on MCC and Halo 2 on Legendary. Twice. Doom had a similar issue but yeah, I know how frustrating it can be. Hopefully this helps.
