Hey does anyone know how to fix this, I go to play the campaign and it says I haven’t even started it yet and mind you I completed the game on legendary by myself and on co-op and now it seems I have to beat it again. Is there any solution to this?
I haven’t had this happen to me, so I can’t be of much help here, but were you connected to XBL when you tried to play Campaign? Also, make sure you were signed in under the same profile used to complete the Campaign.
I know this doesn’t help much, but hopefully it will enough.
BTW, your service record does indeed show that you completed Legendary Campaign both Solo and Co-op, like you said. Connecting to XBL should help correct your issue because of this.

I appreciate your help dude, but funny thing is. I am actually signed into XBL when I checked this. Same thing happen to me on Halo:Reach. With all of the problems I am having on Halo 4, i’m quite frustrated with all the lag bs and random spartan restarts 
Bummer. This happened to you on TWO games?! Wow. That couldn’t have been fun. I agree, my biggest beef with Halo Matchmaking is the amount of lag. I’ve literally put 5 shotgun shells into an enemy player at point blank range, and he proceeded to assassinate me. @$#! lag, lol.
I’m waiting on a reply from 343 on this forum topic. Hopefully they have something to fix this if not, It looks like I have to beat it all over again

I would P.M. a forum admin, such as bsangel, to get this issue resolved. I would also leave this topic up to see if anyone else has had this problem and has a solution for you.
