ive Been playing for a while now and sometimes the triple double medal isn’t issued, last match was 18 assists 10 head shots (two of them perfects) and also 5 saw kills, 5 shot gun kills and another 5 kills with the magum. It seems to be when the connection is bad (which also seems to happen a lot, but that’s for anothwr thread!)
The multikill medals are synced directly to the in-game clock shown at the bottom of the screen, so as you mentioned they can be skewed by poor connection. One time I saw an overkill be continued 12 seconds after the last kill because of extremely poor connection, and I’m sure it would work the other way around as well.
Edit - Just realized you meant the Triple Double medal and not multikill medals whoops.
Triple Double has to be double digit kills, assists and headshots. How many kills did you get? Headshot kills dont count for both, just the headshot.

That’s what I first thought Killer Orca,
As that has happened a number of times before (10+ headshots 10+ assists) this time around I had another 13 kills on top
Strange I never had a medal not show up for me before. It might be due to bad connection not running in sync with the in time clock? Who knows.

Thirteen standard, non-headshots and 10 headshot kills, or 13 kills, 10 of them headshots?

13 standard and 10 head shots total kills 23

I may be mistaken, but I’ve always thought that the Triple Double would only get awarded if you fulfilled the requirements during any type of Slayer match, not during objective gametypes. I don’t know why that little tidbit is solidified into my memory, and I may be mistaken entirely, but I think it came from somewhere legit. That’s really the only reason I can think of (aside from generic, solutionless glitches) for the medal not appearing.

I’ve gotten it during CTF so
I might be wrong on this but I don’t think that Perfect Kills are considered headshots when it comes to the Triple Double medal.
