Every time I attempt a coop campaign with someone else, one of us gets kicked from the game as soon as the mission starts. This has happened to me and multiple other people in the r/Halo Discord - it’s currently making it impossible to complete the Halo 3 co-op challenge. Is this a known issue?

Yup, same here. What I did to complete the co-op challenges was to connect my other controller and play in split-screen.
