Banhammer Strikes for nothing!

This happens to me a lot of times while playing that all of a sudden the game decides to get me back to the menu with a message.

LEFT FIRETEAM
Your fireteam has changed.
Returning you to the main menu.

And this conclus with a banhamer strike of 19 minutes, just because the game decide to drag me out of the game i dont now why and im really pissed off right now witht this its goes 3 times now and i have a 35 minutes ban now. I’m adding a clip from xbox dvr → Video Here
