So it would seem that there is one BOB on the 4th campaign mission “Tip of the Spear” that has yet to be found, and I believe at this point it is because of a bug that prevents it from spawning properly. Many people are looking into this and are searching for the BOB, but despite looking high and low and trying pretty much every option, there are no new BOBs to be found.
On HBO, user ‘Lord Friendship’ posted about this problem, and he received a reply from Niles Sankey, lead mission designer for Reach, via Urk. And this is what he had to say:

Now the part that I made bold is what I want to draw attn to. Niles admits that we would have likely seen this BOB that he confirms existing with as much time and effort we have spent trying to find it.
This leads me to believe that there IS in fact a bug of some type preventing this BOB from spawning correctly.
My request is to have someone from Bungie or 343 (someone who knows exactly where this BOB is supposed to spawn) physically, themselves, pop in a production copy of the game into a production version of an Xbox 360 and play TotS until they can confirm that the BOB is in fact spawning where it should be and isnt bugged.
Or, confirm that it IS bugged and hopefully be able to do something about it. And if nothing can be done about it, at least have the decency to tell us about it so we aren’t wasting any more of our time searching for something that will never show up.
Please 343 I beg you, just check into this for us and let us know what is going on!
