I have seen this happen a few times to me but finally isolated the problem.
I don’t know if other people have experienced this or not, but it ends up in you joining an online game by your self.
Normally while the game is preparing to load, right before you hit the loading screen, should your controller get some
interference and lose connection with your xbox one the game will most likely isolate you from the rest of the players and launch
you in an online game by your self.
To avoid this make sure to avoid any of this while the game has found players, and/or is voting and loading the map.

- To be aware of your controllers Battery status.- Avoid Connecting your controller to a usb source.- Avoid disconnecting your controller from a usb source.- Avoid losing signal from your controller to your xbox one.Please re-post in this thread if my solution helped or if you see the same problem of joining an online game alone
