Live service games are online only. So yes. Another Halo 5.
Most likely yes. It’s already confirmed that like Halo 5 all of multiplayer (including custom games) is going to be hosted on dedicated servers. I don’t know about campaign, but I would anticipate them requiring you to be online. What would really suck is if they made campaign be all on dedicated servers and then you had to have a gold subscription to play it.

Halo 5 didn’t require an internet connection for the solo campaign but did require it for online coop. Halo 4 was the same way but had splitscreen like every other game before it. I’m going to assume that anyone who made it to the forums already knows about Halos 3 to 1 at this point.
Now, I could be wrong since the game isn’t out yet, but I have absolutely no idea why the local campaign features wouldn’t be entirely offline. There are no advantages to it being entirely online, that’s just a gross misuse of server resources. But hey, this is 343i. Anything can happen with a little imagination and dangerous curiosity.
