Yeah, you have to finish all 5 levels of all the commendations to get the helmet, and 3 levels to get the armour, so yeah its pretty rare.
Please don’t wear it. That will mean it’s not exclusive anymore.

You need to go to the Halo 5 Service Record, then Requisitions.
