Hey Guys,
I finished all TMCC Campaign missions in Co-Op yesterday and didn’t get the achievement for the Halo 4 mission “Midnight”. Although I got the achievment for finishing Midnight over 25k points. So I started “Midnight” again in Singleplayer, finished it and didn’t recieve the achievement for finishing the mission again. Because of this I’m also not getting achievements like “finish all missions”.
Any thoughts?

I know what you are going through because I’m in the same situation but with Halo CE part of the game.
I have completed each level but did not get credit for completing the level “Two Betrayals”. I was wondering what was happening when I noticed that other achievements were not unlocking at the time. It seems like what was stopping achievements from popping was fixed by Xbox I assume because what I should have unlocked but didn’t, turned out to have later been awarded to me later except for this one level.
I played it through twice on Legendary and shows that in the menu screen completed for Solo on Legendary, yet No achievement. I was unlocking other achievements at the time when I decided to play it again for a third time with the clock timer going and run through fast on easy. At the end of it I had unlocked the achievement for beating the level under par time but still did not get the additional achievement for just completing the level which is preventing me from unlocking beat all levels achievement too.
I don’t know what to do and I’m quite frustrated about the whole ordeal. Not the first time halo achievements gave me problems…
Anyone know who to talk to or how to fix this issue?
the same is happening to me i contact Xbox about it and they said that it was on 343 side of things so i guess we will have to wait unfortunately but i hope it fixes soon 
Alas, the same happened to me as well on Reclaimer. I needed to play it twice on legendary for it to count for achievements and on waypoint - even though the game itself showed that I had completed the level on solo legendary.
Hopefully the Wednesday patch fixes this as I still have a few stuck on “Done! Unlocking…” and probably many more just not registering at all. Heck i’ve even had random achievements un-related to what i’m doing unlock. It’s very bizarre so hopefully wednesday’s patch fixes most of the big issues players are currently having.
